{"id":92,"date":"2004-12-23T16:07:34","date_gmt":"2004-12-23T21:07:34","guid":{"rendered":"http:\/\/blogs.law.harvard.edu\/neurovista\/2004\/12\/23\/radiotherapeutic-factors-in-the-ma"},"modified":"2006-11-23T23:06:50","modified_gmt":"2006-11-24T04:06:50","slug":"radiotherapeutic-factors-in-the-management-of-cervical-basal-chordo","status":"publish","type":"post","link":"https:\/\/archive.blogs.harvard.edu\/neurovista\/2004\/12\/23\/radiotherapeutic-factors-in-the-management-of-cervical-basal-chordo\/","title":{"rendered":"Radiotherapeutic Factors in the Management of Cervical-basal Chordomas and Chondrosarcomas"},"content":{"rendered":"<p>Noel et al.\u00a0 Neurosurgery December 2004 issue.<\/p>\n<p>The authors reviewed 90 patients who were treated with a combination of high energy photons and protons using the 201-MeV proton beam of the Centre de Protontherapie d&#8217;Orsay.\u00a0\u00a0 There were 64 Chordomas and 64 Chondrosarcomas.\u00a0<\/p>\n<p>There is often residual disease after resection and a relatively poor sensitivity to 50-60 Gy standard doses of radiation.\u00a0\u00a0<\/p>\n<p>Authors decided to explore the control rates using proton therapy.\u00a0 The general rationale is to take advantage of the &#8220;Bragg Peak&#8221; of &#8220;proton therapy&#8221; and attempt\u00a0dose escalation\u00a0to deliver a higher dose, making use of the rapid lateral fall-off of dose (approximately 15% per mm) that is associated with proton therapy.<\/p>\n<p>Local control for chondrosarcomas was 80% at 42 months and 50% for chordomas.\u00a0<\/p>\n<p>In the multivariate analysis, local control and the gross total volume of tumor excluded from the 95% isodose line\u00a0were significant prognostic indicators.<\/p>\n<p>The averate GTV was 29 cubic cm.\u00a0 The GTV excluded from the 90% isodose line was 3 cc.\u00a0 The GTV excluded from the 95% isodose line was 4.4 cc.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Noel et al.\u00a0 Neurosurgery December 2004 issue.
