However, in reading about the last four \u2013 what are now called \u201cadditional\u201d levers \u2013 I was wondering if there might be a better way to categorize or label them.  All the other levers fit into one of three categories: intrinsic; extrinsic; and intrinsic\/extrinsic.  Based on my limited knowledge, it struck me that cost and exit\/entry might fit into the \u201cextrinsic\u201d box, while leadership and efficacy might fit into the \u201cintrinsic\/extrinsic\u201d box.  Is this right?
