Accident investigators attributed the disaster to an undetected<br \/>\n        breach in one of the shuttle&#8217;s wings at launching and a lax safety culture<br \/>\n        at NASA, and they stipulated 15 recommended reforms that must be carried<br \/>\n        out before the fleet flies again.<\/p>\n<p>NASA officials said they had met 5 of the 15 recommendations and would resume<br \/>\nflights only when the benchmarks were met, not because of a time schedule<\/p>\n<p align=\"left\">from the <a href=\"http:\/\/www.nytimes.com\/2004\/10\/02\/national\/02shuttle.html?ex=1254456000&amp;en=4ad6876a2ddf6b5d&amp;ei=5090&amp;partner=rssuserland\">New<br \/>\n      York Times<\/a><\/p>\n<\/td>\n<\/tr>\n<\/table>\n","protected":false},"excerpt":{"rendered":"<p>WASHINGTON, Oct. 1 &#8211; NASA has abandoned plans to resume flying the space shuttle next March because of the impact of four hurricanes on space agency centers and work schedules, officials said Friday.
